There are certain moments we spend a great deal of time planning for in life: the birth of a child, a wedding, a new job. However, one eventuality often gets swept under the rug – our own death. Just as parents create birth plans to ensure they are well supported during labor, it’s equally important to articulate our wishes for our final days.
This free End-of-Life Preferences Worksheet is a simple yet useful tool to ensure your desires are respected and communicated when the time comes (It is also important that you plan formally with advanced directives/wills). This is simply a tool to guide your caregivers and loved ones.
It is never too early to plan and you can always update your decisions as life changes. Pick a date that works well for you to review your plans at least once a year (I use my half birthday!)
